Eligibility criteria
You must be:

  • a school leaver or

  • aged between 16 and 24 (at the start of training)


You will also need to be:

  • living or working in NSW

  • an Australian or New Zealand citizen, permanent resident or humanitarian visa holder

  • signed out of school

  • Not currently completing a traineeship or apprenticeship

Application for Statement of Attainment in Early Childhood Education and Care (CPR, Basic Emergency Life Support and First Aid in an Education/Care Setting)

Please complete all questions and attach two (2) DIFFERENT forms of ID where requested:

An Australian government issued PHOTO ID showing your residential address:

  • Drivers licence or;

  • NSW Photo Card

ID # 2

  • A Birth certificate or;

  • A passport or;

  • A Medicare card or;

  • A naturalisation Certificate or;

  • Certificate of evidence of resident status (CERS)


Please note: STUDENT CARDS, PRIVATE HEALTH INSURANCE CARDS, GYM MEMBERSHIP CARDS, BANK CARDS, KEY CARDS OR CREDIT CARDS CANNOT BE USED AS ID

Please note that training is available in public courses at Campbelltown (NSW, 2560)
or via private group training in Greater Sydney only.

I, the undersigned, understand and agree that personal information (information or an opinion about me), collected from me, my parent or guardian, such as my name, Unique Student Identifier, date of birth, contact details, training outcomes and performance, or sensitive personal information (including my ethnicity or health information) (together Personal Information) collected by Brainwave Learning Centre PTY LTD, trading as Wave Learning may be disclosed to the Department of Education and Communities (Department). The Department may disclose my Personal Information to other Australian government agencies, including those located in States and Territories outside New South Wales. The above government agencies may use my Personal Information for any purpose relating to the exercise of their government functions, including but not limited to the evaluation and assessment of my training, the determination of my eligibility to receive subsidised training or for any Fee Exemptions or Concessions. My Personal Information may also be disclosed to other third parties if required by law. I consent to the collection, use and disclosure of my Personal Information in the manner outlined above. I also acknowledge and agree that the Department may contact me by telephone, email or post during or after I have ceased subsidised training with Brainwave Learning Centre PTY LTD, trading as Wave Learning for the purposes of evaluating and assessing my subsidised training.
